Summary of steps for those not needing insurance coverage for the HST and treatment

  1. Purchase this link.

  2. Issue the screening form for the patient Dr. Zelk uses.

  3. If you are not intending to have insurance coverage for the patient, issue the HST to the patient. We prefer a minimum of two nights of studies.

  4. Send the patient screening to Dr. Zelk and the HST recordings, and schedule them to consult with Dr. Zelk to evaluate, interpret, and issue a prescription for treatment. Here is the consultation link to schedule with Dr. Zelk: https://calendly.com/joe-smn

  5. Dr. Zelk does the consult, consultation notes, interpretation, LMN, and a prescription for treatment is secured from him or another MD licensed in the state where the patient lives.

A board-certified MD will do this telemedicine consult, as will Dr. Joseph Zelk and/or other board-certified sleep medicine MDs in your office or at their home as needed. These medical doctors work with Sleep Balance Academy because they align with the aims of SBA to reach those suffering at a reduced cost and with reduced complexity for patients. We also partner with the Air Institute for high-quality and efficient care because these physicians, chiropractors, and dentists are all aligned with prescribing, when appropriate, the Newest combination therapies most sleep specialists have no familiarity with. Joseph Zelk, DNP, FNP, BC, CBSM, DBSM

Dr. Zelk is a board certified sleep specialist. He is passionate about furthering the implementation of home sleep diagnostics, home CPAP titration and training medical and dental clinicians in mandibular advancement therapy. Dr. Zelk has championed the use of combination therapies to overcome the dependance on CPAP treatment and optimize the outcomes of obstructive sleep apnea solutions.

He is actively working in developing new patents and FDA approved devices in the area of OSAS treatments.
