This item is for those practitioners who dispense home sleep studies from their office to the patient, or for patients who do not have a sleep study completed. This item is for all practitioners in states other than Washington, Oregon, Arizona, or Colorado. If you practice in these four states, use the other ordering link here. If you or your patient wants the sleep study and the treatment for sleep apnea to be covered by Medicare and other health insurance plans, you should use this link. You CANNOT do a sleep study on the patient BEFORE the sleep specialist does the telemedicine consult to determine if they need a sleep study and issues a prescription for a sleep study. Once you have that prescription, you can then issue a sleep study to the patient if your state allows you to.
Once you have completed the prescription-authorized sleep study, a second telemedicine consultation is conducted with the sleep specialist to determine if another prescription can be issued for treatment. If you do not do this in this order, neither your sleep study nor the subsequent treatment will be covered by Medicare. Many other insurance plans have the same requirement.

Joseph Zelk, DNP, FNP, BC, CBSM, DBSM
Dr. Zelk is a board certified sleep specialist. He is passionate about furthering the implementation of home sleep diagnostics, home CPAP titration and training medical and dental clinicians in mandibular advancement therapy. Dr. Zelk has championed the use of combination therapies to overcome the dependance on CPAP treatment and optimize the outcomes of obstructive sleep apnea solutions.
He is actively working in developing new patents and FDA approved devices in the area of OSAS treatments.
