Medical Director Monitoring Program for Sleep Health Services Only

This item includes automatic membership in the SBA program learning modules and is a required product to get telemedicine services in California. We will add states as Dr Zelk becomes available in those states. This monthly ongoing fee is $150.00 per month. The initial set-up fee is a one-time $500 fee, and that will be sent separately. You must maintain your membership in the SBA to qualify for telemedicine services in the program. If you want the medical director program that oversees an in-office nurse who does injections, performs nightlase, and provides other services over and above sleep disorders services, choose the SBA Medical Director Program for expanded services.
